These are basic instructions regarding the electronic forms we produce. Most of the forms are in the Adobe PDF format and require Adobe Reader (Acrobat) version 5 or later to open. It is recommended that you install the latest version of Adobe Reader on your computer that will work with your Operating System.

You can download many different versions of Adobe Reader from http://www.adobe.com. If you have an old computer and cannot install a version of Adobe Reader that will open our forms, please let us know and we will see if we can help.
